I hereby am certifying all information provided in this application is accurate and have sufficiently informed myself about the nature of the camp and activities involved. I acknowledge that there are risks, dangers and hazards associated with my child’s participation in the camp including, but not limited to: impact and collision with other participants, instructors or spectators; adverse weather conditions, failure to play safely within one’s own ability; theft, consumption of food and drink. I understand Pacific First Aid Ltd., its directors, employees, volunteers, training partners and instructors (hereinafter referred to as the “Releasees”) are not responsible for any death, injury, loss, or damage of any kind suffered by any person during participation in the First Aid Camp and all related activities of the program, including injury, loss or damage that might be caused by the negligence of the Releasees. Permission is also granted to Pacific First Aid Ltd.. and its affiliates including Directors, Counsellors, and Volunteers to provide the needed emergency treatment prior to the child’s admission to the medical facility.I understands I will be responsible for any medical or other charges in connection with my child’s treatment. The camper is expected to be respectful and considerate towards other participants, counsellors and volunteers. Campers are expected to follow the directions of all counsellors, to stay in close proximity to their counsellors during the Camp and not leave without consent and informing the camp counsellors. If there is a breach of these rules, counsellors will discuss the issues with the camper and/or the legal guardian. Which may lead to withdraw from the reminder of the camp, without reimbursement of any camp fees. I confirm that I have discussed these rules and expectations with my child. Pacific First Aid cannot be held liable due to unforeseen circumstances (such as changes to location & activities due to weather condition etc.) Full course fees will be charged to campers who do not show up and without written notification 5 days prior to the course start date. No fees will be charged if we require to transfer your registration due to course cancellation. By signing this document, I acknowledge that I understand the terms and conditions of the Program and am aware of the risks, hazards and dangers associated with it; I fully understand the terms of this Agreement, which I have discussed with the participant, and I, for myself, and on behalf of the Participant, consent to the Participant participating in the Program and I accept all of the terms of this Agreement.
